Initial Comments:
Based on the findings of an off-site followup survey completed February 7, 2023, Wholeness Home Health LLC, had corrected the deficiencies cited under PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries. The deficiencies were cited as a result of a state relicensure survey completed October 20, 2022.

An offsite follow-up survey conducted on February 7, 2023 found that Wholeness Home Health LLC, had corrected the deficiency cited under 35 P.S. 448.809 (b). The deficiency was cited as a result of a re-licensure survey completed on October 20, 2022.
